The Payroll/Status Change Form is required for all employee changes of information including rate of pay, employment status, title change, location change, shift change, supervisor change, etc. Supervisors must submit this form to the Employee Engagement Office to ensure accurate and timely processing of changes. In the case of a promotion or transfer, a copy of the letter should accompany the change form. Supervisors are responsible for obtaining approval from the Business Office and the President’s Office to ensure the budget has been verified with respect to changes in salary. Pay rate changes are effective at the start of a new pay period.
